New Patient Registration Form
Are you a new patient wanting an appointment?
If you are a new patient and you have booked an appointment, click on the link (hot doc link to registration form)fill in your details and submit New Patient Registration Form. It is an online form, you can use on your smart mobile or desktop.
Alternatively, you can complete New registration-form_doctors-at-station-st-officer and email it to reception at reception@doctorsatstationstofficer.com.au
Transfer of Medical Records Form
Are you a new patient wanting to transfer your medical records from your previous GP or medical centre?
If you are a new patient and would like to transfer your previous medical records to Doctors At Station St-Officer you can complete request at reception, Download and FAX form to 03 5929 8701 or Email Us.
Results
If you wish to access your medical information, please discuss this with your doctor. Patients will be contacted when test results require follow-up prior to the next planned appointment. All other results can be discussed by making an appointment with your doctor.
All information provided by you at Doctors At Station Street-Officer is strictly confidential and stored securely. However, when a referral is made to another health professional your medical information may be provided to that professional for treatment purposes. Please make sure your preferred contact details are current with the reception staff.
Specialist Referrals
These must be done during a consultation with your GP. Repeat referrals are given at your GPs discretion and you may be charged a fee.
Medical Certificates
You must make an appointment to get a medical certificate and, we regret they cannot be backdated.
